I will attach a 10 day itinerary in Ecuador that you can read as an example trip. This covers a great deal of the country, and I personally think it is a great adventure. However, please use this as an example, and we can go around from there. If you want to change some days, extend some stays, shorten other, or change the route in any way, please let me know!

Day1: Arrival in Quito:
Transfer to Hotel, Depending on arrival time, city tour in the afternoon can be arranged.
Day 2: Cable Car (Teleferiqo) Voyage, Afternoon Equator Monument visit (& historic Center in the morning, if the previous day arrival is too late)
Day 3: Day trip to Mindo cloud Forest, return to Quito
Day 4: Head north to Otavalo Region, Visit indigenous markets, and northern lake region. hikes and extra visits can be arranged.
Day 5: Voyage to Cotopaxi National Park. Day Hike in the area. Overnight in local Hacienda.
Day 6: Voyage South to Quilotoa region, visit the Volcano, hike in the area. Afternoon, overland to Baños.

Before presenting prices to you, I would like to ask: What categories of hotels would you like?
do you see yourself joining a group tour, having a private guide, or simply having indications and doing it all by yourself?

and with regards to transportation, do you want to travel in private cars, join in on fixed departure group tours, or use the public transportation around?

that looks really good, I think we're looking for something that's cheaper but safe. We looked at hotels earlier, maybe something in the $60 to 70 range (for the two of us). Let me ask my friend about the rest of the details really quick!

Hi Tim!
First of all some info on the things you can do for yourself without previous reservations!
The transfers to and from the Airport go for $25, in a regular cab (one way), but it is not necessary to make previous reservations, you can grab them at the airport at your arrival safely and easily.
The cable Car (Teleferiqo) ticket price is $9 per person, and a regular cab fee there will be around 4 to 5 dollars from the Fuente de Piedra hotel. These should also be fine without previous reservations.
In Baños, I would recommend at least 2 nights of stay so that you can participate in many of the awesome sport activities such as mountain biking, bungee jumping, quad rental, canyoning, rafting, etc. These activities can be negotiated directly there and available at all times. If you need a catalog of these activities please let me know.

What I can offer you is the following accommodations:
Nightly accommodation in the nice colonial style Fuente de Piedra Hotel, for USD$30 per night per person.
In Machachi, there is a nice colonial style hacienda called La Estacion. We can offer you the price of 35 USD per person if you wish to stay here. (can be reached easily after your visit to Cotopaxi National Park listed below)
In Baños, hotels are a little more expensive, around 45 USD per night in good quality hotels.

I can offer you the following activities within Quito:
Full Day City Tour in double decker bus: $18 USD per person (No Equator visit)
Full Day city tour with Equator Line in regular tour bus: $33 per person.

The following Day Tours are really nice and highly recommended: (Price Per Person)
Mindo Full Day Tour $90 USD
Otavalo indigenous markets: $70 USD
Cotopaxi National Park: $80 USD

Transfers to Baños costs $150USD on a private car that fits up to 4 people. This transport service will also stop in the "Pailon del Diablo" waterfall, and the Agoyan Cable Car.

If you would like me to handle the reservations for the hotels, then we would require a payment as soon as possible.

short answer to your questions:
Yes, the transport can take you to Quilotoa, the driver will pretty much take you wherever you want to go this day, and it is somewhat on-route.
The hotel will certainly accept the check in, we have asked them to have early check ins (if possible according to previous night's occupation) for all Kim Kim clients, so no worries about this.

The quote looks great and we just had a few questions before we finalize it.

1. Are we going to be given a location as to where to catch the double decker bus?
2. Are we provided transportation to the cotopaxi national park?
3. On the document, it says that we have a reservation at the local hacienda near cotopaxi. However, for the following day, it says that were are getting picked up by a driver from quito. Is there anyway for us to get picked up at the La Estacion Hacienda instead?
4. On the document it says, that we are returning to quito from banos on our own. Do you have any safe-reliable transportation methods that we could use? If not, how much would a private transport cost?
5. Is transportation provided to the Mindo forrest or are we responsible for getting their on our own?

Hi Tim!
I apologize I did not give you the inclusions for the Mindo and Cotopaxi Tours, but here is the answer to your questions:
1) Certainly! when you arrive to Quito, you can visit our office and we will give you the ticket and voucher for the double Decker bus, (or we can drop this off at the hotel, up to you), as well as maps of the stops and any other information you might require.
2)For the Cotopaxi National Park Day Trip, the following is included in the shared tour: Transport from Quito to the park and back, Lunch in Latacunga, a Guide & the walks in the park, and a small snack, all included in the price. The driver will drop you off in La Estacion, in Machachi town, after the visit.
3) I apologize, that was my mistake. Since it is private transportation, they can pick you up from anywhere. In fact it would be better for you and the driver to pick you from La Estacion, since it is on the way to Quilotoa, thus saving you some driving time this day.
4) The transport back on private basis is also $150 USD. There are also safe and reliable alternatives with local buses, that will drop you off in the terminal, from where you can take a taxi.These range in price from 4 dollars to 15, and go off really often (in some days, there are departures from Baños to Quito every 30 minutes)
5)Similarly to the Cotopaxi Day tour, you will have a departure from a set point in Quito, where transport will take you to Mindo. Here you have the activities of Zip Lining, Canoping, the visit to a "Butterfly Green House", and a snack, all included in the price.

On the day trips, of Cotopaxi and Mindo you are responsible of getting to the departure point and place on time, which will most likely be a short taxi ride away from your hotel. Some money for extra food, or souvenirs is recommended!

Would you be able to drop off the ticket and voucher for the double decker bus at our hotel please? Also, just to clarify, the hotel is okay with us checking in around 9am as our flight lands at 7am? Could we also get the addresses of the departure points to the cotopaxi park and mindo forrest? As for transport back to Quito, we decided to use the public transportation option and were wondering if we could get more information about that? Lastly, what methods of payments are accepted at the national parks (cards vs cash)?

No worries about asking, in fact please do!
We will leave everything ready and will notify the Hotel about your arrival, and if the rooms are ready at the time you get there, then they will give them out immediately to you. At the moment I cannot tell you if they will be ready, because it will depend on the previous night's occupation. If they are not, then you can leave your baggage in a safe storage in the hotel itself.

The departure to the day trips (Mindo and Cotopaxi) will be from the same place: Plaza Foch (Reina Victoria & Mariscal Foch), at 7:15 am (recommended you arrive 7 at the latest). This is less than 6 blocks away from your hotel.

The public transportation option departs from the Baños bus terminal. They generally leave every hour, or on the 30 minute mark after every hour. Depending on the time you want to leave then it would be recommended to head there with some spare minutes. Unfortunately there is no option to buy these tickets online or previously, but I could give you recommendations of specific carriers that you might prefer using.

In most restaurants in Ecuador, Card is accepted fine (visa mastercard, Amex, etc.) Same with hotels.
With regards to: Taxis, souvenirs, national parks, and more: Cash ONLY, otherwise you might run into some problems.

Last question, is the hotel in Quito called Fuente de Piedra Hotel I or II as there are two different hotels coming up on google maps. Thank you!

Absolutely! the carrier I can recommend is Transportes Baños, who have daily departures at 14:00, 14:30, 15:50, 17:00, and many more. The ride will take around 4 hours. You will arrive to the terminal from where you can simply take a cab to Foch and the Fuente de Piedra Hotel.
For the Hotel In Quito, we regularly use both hotels. Fuente de Piedra 2 is closer to Foch Square. This is where you will stay.

Baños is also a small town, so finding the bus terminal should not be a problem, any taxi there can give you a ride for a few dollars. The departure for the day Trips is at the "Magic Bean" Restaurant, please be there at 7:00 am.
