From the pics I have seen so far - the Annapurna region looks what I would be most interested in - though with the short time I have I'm not sure if its doable.
If you can arrange a guide, that would be great. I'd prefer a female guide, if available.

So you have 3 days of actual trekking... not sure if Pokhara makes sense then. There are also some good options around Kathmandu. Since it's night in Nepal now, it will take a few hours to get back.

I have noted that you are interested in Annapurna region for short trekking. Please find below the proposed program of Annapurna region short trek.
Annapurna region trek
Day 01 (Sat) - Fly Kathmandu - Pokhara in the morning. Drive to Phedi and trek to Pothana. Overnight at local lodge.
Day 02 (Sun) - Trek to Ghandruk. Overnight at local lodge.
Day 03 (Mon) - Trek to Birethanti and drive back to Pokhara. Overnight at hotel.
Day 04 (Tue) - Early morning fly back to Kathmandu.
As an another option, you can also do hiking trip in Kathmandu valley which will have less rush than Annapurna region trek. You can have magnificient views of Himalaya range from Nagarkot view tower and enjoy the trek at the same time. The program itinerary of Kathmandu Valley Trek is as hereunder :
Kathmandu Valley Trek
Day 01 (Sat) - Drive to Sundarijal. Trek to Chisapani.
Day 02 (Sun) - Trek to Nagarkot.
Day 03 (Mon) - Enjoy sunrise view from Nagarkot view tower. Trek to Dhulikhel.
Day 04 (Tue) - Early drive back to Kathmandu.

One more question. ..for the visa on arrival process, do I need to fill anything online or bring any documents or would everything be done at the airport?

Since recently you can apply for a Nepal tourist visa online: http://online.nepalimmigration.gov.np/tourist-visa
Alternatively you can do it at the airport when you arrive. If you decide to do it at the airport you should bring 4 passport sized photos with you and just in case a couple of 100 USD in $20 bills. (although the last time I was there a couple of years ago I didn't bring photos and you can have these taken at the airport as well).
